Incident Reporting

The Incident Reporting procedures of CHaOS Rowing:

IN CASE OF INJURY, CONTACT 911 IMMEDIATELY.
All incidents (actual accidents, damages, and near misses) involving CHaOS members and/or CHaOS equipment MUST be reported through this form or by e-mail to safety@chaosrowing.org.

Please structure your incident report along the following questions:

what happened?
who was involved?
why did it happen?
how could it have been prevented?
what are the lessons learned?

If appropriate, anonymized incident reports – especially the lessons learned - will be shared with rowers and guests.

These can be things not necessarily related to the incident itself. For example, when you flip under the bridge, the only place to re-enter is the sandy beach on the north-west of the bridge. The rocks along the road dam are too steep.
